Course Forms
- Outside Course Approval
Form
MIS students may count up to 6 hours outside SLIS toward the MIS degree.
Specialist (post-master's) degree students may count up to 15 hours outside
SLIS with approval. These outside courses must be approved by the student's
adviser.
- Course Waiver Approval Form
If course waiver is approved, the number of hours required for the degree
remains the same. The waiver allows students the option of an extra elective
course rather than a required core course.

- S601 (L600) Directed Readings Proposal Form
Allows for students to study a specific field of specialization in which there is not
enough interest to warrant regular classroom instruction.
- S602 (L594) Directed Research Proposal Form
Allows for students to develop or conduct research projects under faculty supervision. These
research projects are often extensions of work begun in a course, or in some other way build
on preparation received during studies at SLIS.

Graduation Guidelines
- Graduation
Application (Masters & Specialists)
This form is for SLIS degree processing. Students should submit forms to the
SLIS Office for May, June and August graduation by March 1; and for December
graduation by September 15. Ph.D. students should contact Arlene Merkel.
